GATE CARD and KEY APPLICATION

To acquire a Gate Card and/or Key, you must fill out a Key Request Form. The form can then be handed in at the Main Office in B478 or scanned or emailed to mordonez1@berkeley.edu. The form must be signed by your Department Key Approver or Manager.

To receive a Gate Card and/or Key, recipients are required to complete the “Working at RFS Training and Safety Module” and pass the short quiz that follows. The RFS office must receive confirmation that the module quiz was completed and passed before keys are issued.
